Qualifying conditions for a medical cannabis card in Iowa (2026)
Iowa's Medical Cannabis Program, officially the Medical Cannabidiol Program, lets patients with specific debilitating conditions legally buy state-regulated cannabis products from licensed dispensaries. The Iowa Department of Health and Human Services (HHS) runs the program. To enroll, you need a written certification from an Iowa-licensed healthcare practitioner confirming that you have at least one condition on the state's approved list.
What changed in 2026
On July 1, 2026, House File 990 took effect and made two major changes to the program. The list of qualifying conditions did not change. The 4.5-gram THC purchase limit and the ban on smokable flower also remain in place.
Residency is no longer required
Patients from outside Iowa can now apply for an Iowa registration card, as long as an Iowa-licensed practitioner certifies them.
More dispensaries are coming
The law doubles the number of licensed dispensaries from five to ten. That means shorter drives and better access for patients across the state.
Why get a medical card in Iowa?
Recreational cannabis is illegal in Iowa. Outside the medical program, possessing any amount of marijuana is a criminal offense.
A registration card is the only legal way to buy and possess cannabis products in the state, and it protects registered patients and caregivers from state prosecution when they follow program rules. Cardholders also get lab-tested products with consistent dosing from state-licensed dispensaries.
Iowa's 13 qualifying medical conditions
You need at least one of these, certified by an Iowa-licensed practitioner.
Don't see your condition?
Iowa's list is shorter than those of many states, but it can grow. The Medical Cannabidiol Board reviews public petitions to add new conditions, and the Board of Medicine must approve any additions. Many symptoms, especially ongoing pain, may already fall under an existing category such as chronic pain. If you're unsure whether you qualify, a practitioner can review your records and tell you.
